PCN Occupational Therapists

Our PCN (Primary Care Network) Occupational Therapsists work within a multidisciplinary primary care team to support patients in optimizing their daily lives and achieving their individual goals. They assess patients needs, provide interventions to address barriers to participation, ad facilitate referrals to other services if needed.

They can help with a number of issues, such as mental health & wellbeing support, cognitive assessments, falls prevention and return to work support.

If you feel an occupational therapist may be able to help you, then you can get in touch with the GP surgery for a referral.
