Social Prescribing Link Workers (more commonly known as Social Prescribers) help people to take control of their own health & wellbeing. They take the time to listen to the person and focus on “what matters to me?”
They take a holistic approach by connecting people to community-based support. This could be local or online social groups, activities or workshops, and the outcome is to help people meet their practical, social and emotional needs. They can also assist with housing, financial and welfare advice.
Northwich currently has four social prescribers, Donna, Lauren, Wendy & Karen.
