Non – NHS / Private work. Why are fees charged?

The government’s contract with GPs covers medical services to NHS patients. However, in recent years more and more organisations have been involving doctors in a wide range of non-NHS work. This work is not funded by the government, so GPs have to charge a fee to cover their time and other expenses.

Surely the doctor is being paid anyway?

It is important to understand that many GPs are not employed by the NHS. They are self-employed and they have to cover the costs of everything from the limited NHS funds provided – staff wages, buildings, heating, lighting, etc. – in the same way as any small business. The NHS covers costs for NHS work, but not for non-NHS work, the fees charged by GPs contribute towards keeping the surgery running.

Do GPs have to do non-NHS work for their patients?

With certain limited exceptions, GPs do not have to carry out non-NHS work. Many GPs however will always attempt to assist their patients and carry out this work.

Why does it sometimes take my GP a long time to complete my form/letter?

Time spent completing forms & preparing reports takes the GP away from the medical care of patients which will always have top priority. GPs have ever increased workload of forms which must be prioritised against offering appointments and other urgent administration. We aim to complete non-NHS work within 28 days of receipt.

I only need the GP’s signature – what’s the problem?

When a GP signs a certificate, completes a report, or writes a letter it is a condition of remaining on the medical register (which allows them to practice as a doctor), that they only sign what they know to be true. To complete even the simplest of forms, the doctor may have to check a patient’s entire medical record.

Turnover time

Because NHS work always takes priority, private requests are always completed outside of normal working hours of our staff and clinicians this means that the waiting time for completion of the document / certificate / form may be long and our current standard is 6 weeks.

PCN Occupational Therapists

Our PCN (Primary Care Network) Occupational Therapsists work within a multidisciplinary primary care team to support patients in optimizing their daily lives and achieving their individual goals. They assess patients needs, provide interventions to address barriers to participation, ad facilitate referrals to other services if needed.

They can help with a number of issues, such as mental health & wellbeing support, cognitive assessments, falls prevention and return to work support.

If you feel an occupational therapist may be able to help you, then you can get in touch with the GP surgery for a referral.

Primary Eye Care

Primary Eyecare Services offers a range of specialist NHS eye care services, delivered by accredited optometrists, with appointments at locations and times convenient for you. Service availability is dependent on which services each NHS region has commissioned in their local area.

There are a range of service that they cover such as urgent or minor eyecare services.

Talking Therapies

Talking Therapies offers a range of talking therapies, advice and information for people aged 16 and over, who have mild to moderate symptoms of anxiety & depression.

It is a free and confidential service, designed to help with common mental health concerns, such as:-

  • Low mood
  • Anxiety or worry
  • Stress
  • Depression

First Contact Physiotherapist

Across our GP practices in Cheshire we’ve recruited extra healthcare professionals at our GP practices. In your GP practice you may have a First contact Physiotherapist who is able to offer quick access to expert physiotherapist assessment.

Social Prescribing Link Workers

Social Prescribing Link Workers (more commonly known as Social Prescribers) help people to take control of their own health & wellbeing. They take the time to listen to the person and focus on “what matters to me?”

They take a holistic approach by connecting people to community-based support. This could be local or online social groups, activities or workshops, and the outcome is to help people meet their practical, social and emotional needs. They can also assist with housing, financial and welfare advice.

Northwich currently has four social prescribers, Donna, Lauren, Wendy & Karen.

Clinical Pharmacist

Across our GP practices in Cheshire we’ve recruited extra healthcare professionals at our GP practices. In your GP practice you may have a Clinical Pharmacist who can review your medications to make sure they are right for your condition which will improve your well-being and quality of life.

MSK Practitioner

MSK practitioners are first contact physiotherapists operating at an advanced level of clinical practice, with skills to assess, diagnose, treat, and manage musculoskeletal (MSK) problems and undifferentiated conditions.

This will involve seeing patients, without prior referral form from their GP, to establish a rapid and accurate diagnosis, including ordering imaging, developing a management plan, and referring, if necessary.

They work independently in clinical practice and do not require day to day supervision.

Think Pharmacy – Minor Ailments Scheme

At Firdale Medical Centre we have access to Community Pharmacy Services and will often signpost our patients to their services.

Pharmacists play a key role in providing quality healthcare to patients.  Working in the community, pharmacists use their clinical expertise together with their practical knowledge to ensure the safe supply and use of medicines by patients and members of the public.

Many pharmacies in the local area offer the ‘Think Pharmacy’ service. This service is an alternative to visiting your GP, making it easier for you and your family to get healthcare and advice on minor ailments at a time that suits you.

Your local pharmacy can offer advice and if necessary, prescribe medication to treat the following ailments under the Minor Ailments service:

  • Impetigo
  • Oral thrush in babies
  • Scabies
  • Threadworm
  • Vaginal thrush

Pharmacists can also offer advice on common problems such as coughs, colds, aches and pains as well as healthy eating and stopping smoking.

You can talk to your pharmacist in confidence, even about the most personal symptoms and you don’t need to make an appointment; most pharmacies now have a private consultation area where patients can discuss issues with pharmacy staff without being overheard by other members of the public.

Consultations are always free and confidential, regardless of whether the pharmacist gives you any medication.

Test Results

Results of the tests requested by our clinicians will automatically return to the practice and will be saved in your records. If you haven’t heard from us after your test there is nothing to worry about and the results are fine. If you require a further appointment, you will be contacted by the surgery.

The best way to access your test results as any other information we hold on your records is via NHS App which will give you access to all your medical records including test results and documents. You can also find out what your results were by submitting Admin Request via our Online Consultation Platform AccuRx. This is our preferred method of contacting the practice as it is easy to use, provides with clear record of your request and allows our team to manage our workload more effectively.
